In detail
What medical chronology means in practice
A chronology converts an unordered record set into a readable treatment history. Useful chronologies separate objective findings from subjective complaints and preserve the page citation for each entry.
Why it matters
The chronology is what makes the treatment narrative verifiable. Without citations, a reviewer must rebuild the timeline themselves — and will read any ambiguity conservatively.
Example
03/14 — Orthopedics, positive Spurling test, cervical MRI ordered (p. 214).
