- Records requested from every provider named anywhere in the file

Timeline
- Initial treatment date relative to the incident is documented
- Intervals over roughly 30 days are explained in the narrative or in the record
- Escalations to specialty care are shown in sequence
- Discharge, plateau or MMI documentation is present where relevant

- Specials total equals the sum of attached billing
- Adjustments, write-offs and liens identified
- Wage loss supported by employer or payroll documentation
- Out-of-pocket items supported by receipts
- Future care supported by a provider recommendation and a cost basis
Narrative and exhibits
- Each factual assertion carries a citation
- Exhibit index matches actual pagination
- No figure appears in the letter that differs from the summary tables
- Response deadline and delivery method stated
